An orthopedic sales rep sells implants and instruments for bones, joints and soft tissue to surgeons and hospitals, and spends much of the week in the operating room supporting cases. It is one of the most sought-after seats in medical sales, and most people enter it through an associate role. On 1 November 2026 this board had 320 open orthopedic sales roles at device makers, 241 of them in the United States, and about a third of the US seats were associate roles. This guide covers what the job is, the specialties, the associate path, what the postings pay and how to get hired.
The roles counted here are the open roles at medical device makers whose titles name an orthopedic line (trauma, joint replacement, sports medicine, spine, foot and ankle, extremities and CMF), read from each company's own careers page: Stryker, Smith+Nephew, Johnson & Johnson MedTech, Medtronic and Globus Medical on 1 November 2026. The orthopedic sales jobs page lists every open seat.

1. What an orthopedic sales rep does
- Covers cases. In the operating room, the rep makes sure the surgeon and the team have the right implant sizes and instruments, and answers questions about the system during the procedure.
- Manages the inventory. Implant sets are often held at the hospital on consignment; the rep tracks what is used, what must be replaced and what is billed.
- Sells. Wins surgeons and hospitals over to the company's systems, and keeps existing surgeons using them, against competitors whose reps are often in the same hospitals.
- Is on call. Trauma seats cover fractures that arrive as emergencies, at night and on weekends; joint replacement, spine and sports medicine follow scheduled surgeries more often.
2. The specialties
| Specialty | US open roles, 1 Nov 2026 | What it sells |
|---|---|---|
| Sports medicine | 61 | Implants and devices for arthroscopic and soft tissue repair |
| Trauma | 57 | Plates, screws and nails for fractures |
| Foot and ankle, and extremities | 57 | Implants for the foot, ankle, shoulder, elbow and hand |
| Joint replacement | 28 | Hip and knee implant systems |
| Spine | 11 | Spinal implants and instruments |
| CMF | 7 | Implants for the skull and face |
The counts come from the specialty each title names; 20 more US roles named orthopedics without a specialty.
3. The associate path
Most orthopedic sales teams hire associates first. Of the 241 US roles, 81 were associate sales representative or sales associate roles, and 28 more were summer 2027 internships. An associate covers cases beside one or more senior reps, learns the products and the hospitals, and manages inventory, with the expectation of taking a territory after one to three years; many postings say so. The associate postings that state pay give a base salary, and the territory rep's commission comes later.
4. What the postings pay
Few orthopedic postings state their pay: 25 of the 241 US roles did on 1 November 2026, so these are examples rather than a market band.
| Posting (company) | Posted pay per year |
|---|---|
| Summer 2027 internship, joint replacement ( Stryker) | $41,600 |
| Sports medicine associate sales representative ( Smith+Nephew) | $50,000 to $60,000 |
| Trauma clinical sales specialist ( Johnson & Johnson MedTech) | $54,000 to $87,400 |
| Upper extremities sales associate ( Stryker) | $65,000 to $68,250 |
| Sports medicine territory manager ( Smith+Nephew) | $60,000 to $100,000 |
| Senior orthopedic sales representative ( Smith+Nephew) | $100,000 to $150,000 |
| Capital equipment sales manager, spine ( Johnson & Johnson MedTech) | $114,000 to $182,850 |
| Regional sales manager ( Stryker) | $287,500 |
Territory reps are usually paid a base and commission on their territory's sales, and the commission is rarely in the posting; ask what the reps in the territory earned in total last year. The guide Medical device sales salary explains how device pay is built.
5. Who hires
On 1 November 2026, Stryker had 187 of the 241 US orthopedic sales roles, Smith+Nephew 28 and Johnson & Johnson MedTech 24, with Medtronic and Globus Medical holding the rest. Abroad, Australia (14) and Japan (12) had the most. A large share of the seats are named for a single city or region in the title (Jacksonville, Orange County, Northeastern Pennsylvania), since a territory is usually a set of hospitals within driving distance.
6. Winning the first seat
- Show you can work the hours. Early cases, call and travel are part of the job, so interviewers look for a record of drive and stamina: competitive sport, military service and demanding first jobs are common examples.
- Learn the anatomy and the procedures of the specialty you apply to, and be ready to talk a surgeon through why your system helps.
- Get through hospital credentialing. Vendor credentialing (vaccinations, background checks, hospital training) is required to enter operating rooms.
- Live in the territory. The posting names it, and recruiters expect you there.
The guide How to get into medical device sales covers the interview loop and the other device seats.
7. How these figures were counted
- The roles: every open role on this board on 1 November 2026 at a medical device maker whose title names an orthopedic line, read from each company's own careers system and checked every hour: 320 roles, 241 of them in the United States.
- The specialties and seats: from the words in each title; the pay from each posting's own range on a full year's scale.
These figures describe one day; the orthopedic sales jobs page always shows the current roles.
